What it’s for

The appointment is often short, and your state is hard to describe on the spot. Writing it down saves time and adds precision.

What to write

  1. How long the state has lasted and when it began.
  2. Sleep, appetite, energy — concretely, in hours and meals.
  3. What I can no longer manage that I used to.
  4. Medicines and supplements you take, alcohol included.
  5. Family history, if you know it.
  6. Three questions you want to ask.
