You are standing in a queue in a shop, nothing in particular is happening — and suddenly your heart starts pounding, your palms sweat, your chest tightens. Or you wake up with a sense that something bad is going to happen, though you have no idea what. Only then do the thoughts arrive: “What’s wrong with me? Why is this happening?” The body was first. The thoughts came after it, like someone chasing a train.

That sequence is not an accident and it is not a sign that you are “unhinged”. It is exactly how your brain is built. Anxiety does not begin in the part of the brain you think with — it begins in an older, faster system that sets off an alarm before the reasoning part has even had a chance to assess what is going on.

In this piece I will explain what really happens in the brain and the body during anxiety: why the body reacts before you can think, why calming yourself with reason does not work straight away, and what actually helps in that moment. Because once you understand the mechanism, anxiety loses a large part of its most frightening quality — its mysteriousness.

An alarm that fires before you can think

Deep in the brain, one on each side, lie two almond-shaped structures called the amygdala. Their job is a single one: to check continuously whether there is anything in the environment (or in the body) that might endanger you. It works like a fire alarm in a building — it does not think, it does not weigh things up, it does not wait for further information. When it detects a pattern resembling danger, it fires.

And here is the crucial part: the amygdala is faster than your consciousness. The neuroscientist Joseph LeDoux, who spent decades researching the brain’s fear pathways, showed that information from the senses reaches the amygdala by a shortcut that bypasses the reasoning parts of the brain. That is why your body can be put on standby before you are even aware of what you saw or heard — let alone able to evaluate it.

From an evolutionary point of view this is a brilliant design. Our ancestors who jumped back at a rustle in the grass and only then checked whether it was a snake or a branch survived. Those who analysed first often did not. So the amygdala is calibrated on the principle of “better a hundred times unnecessarily than once too late”. From a survival point of view a false alarm is cheap; a missed danger is fatal. Your anxiety, then, is an alarm doing its job — it is just that in modern life it goes off at emails, at a silence in a conversation or at a quickened pulse, rather than at snakes.

Two roads: the fast shortcut and the slow scenic route

LeDoux described this arrangement with a two-pathway model that is worth knowing, because it explains almost everything that is awful about anxiety.

The fast road runs from the senses via the thalamus (the brain’s “reception desk”) straight to the amygdala. It is extremely fast — in animal studies, a matter of a few tens of milliseconds — but crude: it carries only a rough outline of what is happening. “Something long on the ground!” No detail, no context. Enough to set the body going.

The slow road runs from the thalamus first into the sensory cortex and on into the prefrontal cortex — the part of the brain just behind the forehead that analyses, compares with memory, takes context into account and draws conclusions. This road is accurate, but takes considerably longer. Only it can say: “That is a garden hose, not a snake.” Or: “This is only a meeting, not a trial.”

Between the firing of the fast road and the arrival of the slow one there is a gap — a moment in which the body is already in alarm while reason has not yet had its say. Most anxious experiences live in exactly that gap: a heart pounding “out of nowhere”, a lump in the throat in the middle of a quiet evening, a wave of panic you cannot explain. The body has not gone mad. It has simply got ahead of you.

What the body does in that instant

When the amygdala sets off the alarm, it activates the sympathetic nervous system through the hypothalamus — the branch of the autonomic nervous system that prepares the body for fight or flight. The adrenal glands release adrenaline, and under longer-lasting stress cortisol rises too, via the hypothalamic–pituitary–adrenal axis. Everything you then feel is the logistics of preparing for action:

  • Your heart pounds and your chest tightens: it is pumping more blood into the large muscles so you can run or fight.
  • Your breathing is fast and shallow: the body is stocking up on oxygen for exertion; if the exertion never comes, dizziness and tingling can appear.
  • Your stomach clenches and you feel sick: digestion is superfluous in danger, so blood is diverted from the abdomen into the muscles.
  • Your muscles are tense and you tremble: they are ready for a leap that will not happen.
  • You sweat and feel chilled: the body is cooling itself in advance and redirecting the circulation.

Every one of these signs makes sense — for a sprint away from a predator. But when the same programme starts up in a meeting or at three in the morning in bed, you are left with all that energy and no address to send it to. And because the bodily sensations are strong and unfamiliar, the amygdala readily reads them as fresh evidence of danger: “My heart is racing — something really is wrong!” The alarm sets off the body, the body sets off the alarm. That is how the spiral arises that many people know as a panic attack.

Why you cannot think your way out of anxiety

Now you can also see why well-meant sentences such as “there’s nothing to worry about” or “just calm down” so often ring hollow. They are addressed to the slow road — to the prefrontal cortex. And that is precisely the part of the brain that is at a disadvantage during a strong alarm.

The researcher Amy Arnsten of Yale University has shown that stress chemistry — the flood of noradrenaline and dopamine in acute stress — literally weakens the prefrontal cortex: its connections go quiet, and control is taken over by older, faster structures, the amygdala among them. Put vividly: when the alarm is loud enough, the pilot steps aside and the plane is taken over by an autopilot that knows only one thing — run or fight. Brain imaging in people with anxiety disorders shows a consistent picture: a more reactive amygdala and weaker calming activity in the frontal regions.

So reasoning yourself calm does not work immediately — not because your reason is too weak, but because at that moment it does not have full access to the controls. First the alarm has to be turned down through the body (more on this below), and only then does reason get a word in again. The order matters: body first, then thoughts. When we try it the other way around, we feel even more helpless — and helplessness is excellent fuel for anxiety. Anxiety is unpleasant — but it is not dangerous

This is perhaps the most important sentence in this piece: anxiety is extremely unpleasant, but it is not dangerous. Everything you feel — the pounding heart, the tight throat, the dizziness, the tingling — are signs of a body doing too much, not of a body failing. A panic attack does not cause a heart attack, or suffocation, or loss of sanity. The sympathetic system has a built-in counterweight: the parasympathetic, which always, without exception, calms the activation sooner or later. A wave of anxiety rises, peaks and subsides — even if you do nothing at all.

Why does this matter so much? Because the greatest damage is done not by anxiety itself, but by the fear of it. Once we begin to fear our own bodily sensations, we start scanning the body like a detector: is my heart too fast? is my breathing odd? And attentive searching for danger always finds danger. A step further: we begin avoiding the places and situations where the anxiety struck — the shop, the motorway, the lecture hall. Every avoidance brings instant relief and, in the long run, strengthens the alarm, because the amygdala “concludes” from it: so it really was dangerous there. The circle narrows, life shrinks.

In my work with people I notice that this turnaround — from “anxiety will destroy me” to “anxiety is a horrible feeling that cannot harm me” — is often the first moment when something shifts. It does not do away with the anxiety, but it takes away its second storey: the fear of fear.

The window of tolerance: how much arousal you can carry

The psychiatrist Daniel Siegel introduced a concept I use almost every day in my work: the window of tolerance. It is the range of arousal within which your nervous system works well — you feel, you think, you respond, you stay in contact with yourself and with others. Above the window is the zone of hyperarousal: anxiety, panic, irritability, racing thoughts. Below it is the zone of hypoarousal: numbness, disconnection, fog, listlessness.

In this language, anxiety is simply a sign that you have slipped over the upper edge of the window. And, importantly: the width of the window is not the same for everyone and is not constant. It is narrowed by lack of sleep, hunger, illness, prolonged stress — and above all by past experience. If your nervous system grew up around unpredictability, or if trauma has marked you, the alarm may be permanently set more finely; how such experiences reshape the nervous system, and what the window of tolerance means with trauma, I wrote about in my piece on the myth of “what doesn’t kill you makes you stronger”.

The good news is twofold. First: the window can be widened — through experiences of safety, through practice, through a relationship in which the nervous system can recalibrate. And second: even when you do slip out of the window, there are ways to bring yourself back. Not through reason — through the body.

Two techniques that speak to the alarm in its own language

The first is breathing with a lengthened out-breath. Breathing is the one function of the autonomic nervous system you can direct consciously — which makes it the bridge between will and alarm. On the in-breath the heart rate speeds up slightly, on the out-breath it slows; the out-breath is the parasympathetic’s moment. So if you breathe in such a way that the out-breath is longer than the in-breath — for example in through the nose for four seconds, out through pursed lips for six to eight — you are sending your body the message “it is safe”, minute by minute. Research on slow breathing consistently shows a rise in heart rate variability and a settling of the stress response. Do not expect a miracle within three breaths; count on a few minutes. And practise when you are calm — in the middle of a storm the hand reaches only for what it is used to.

The second is 5-4-3-2-1 grounding. When anxiety pulls you into a whirl of thoughts about the future, deliberately return your attention to the present through the senses: find five things you can see, four you can hear, three you can touch, two you can smell and one you can taste. The exercise is not a trick for distraction — it is a message to the amygdala that the surroundings have been surveyed and are safe. The alarm feeds on the future (“what if”), while the senses live only in the now. And a danger that is not there cannot hide in the now.

Both techniques share the same logic: you do not argue with the alarm using arguments it cannot understand, you speak to it in a language it does — through the body and the senses. Only once the activation has come down is it time for thoughts: what happened, what was the anxiety trying to tell me, what do I need.

When anxiety is more than an alarm — and where to find help

Occasional anxiety is part of a healthy life; without it you would not cross the road attentively or turn up prepared for an exam. We speak of an anxiety disorder when anxiety becomes a constant companion, when you start avoiding it in ways that shrink your life, or when the physical symptoms and the worrying noticeably eat into your day, your work and your relationships. At that point there is no reason to wait — anxiety disorders are among the best-treated forms of mental distress, and psychotherapy is remarkably effective with them.

In therapy we usually work on both sides: the bodily one (widening the window of tolerance, working with the breath and with sensations in the body — my training in body-oriented psychotherapy comes in useful here) and the content one: why the alarm is so sensitive, which experiences calibrated it, what in your life today genuinely demands attention. Anxiety is often a messenger — it points to being overloaded, to a relationship that takes, to needs that have been waiting too long.
