You knew it would hurt. But you didn’t expect this: not being able to eat. Waking at three with your heart pounding. A wave of panic washing over you in the middle of a meeting, for no reason. A quite physical, heavy pain in your chest — as though someone had laid a stone there. And your body, which was working normally only yesterday, suddenly a foreign country.

If you are wondering, after the end of a long relationship, whether you are losing your mind — you are not. Your body is doing exactly what bodies do when they lose the person they have been calibrated to for years. A separation is not only an emotional event; it is a physiological event of the first order.

In this piece I explain what happens in your nervous system when a long relationship ends — why sleep, appetite and concentration fall apart, why it literally hurts in your chest and, most importantly, how to help the system settle again. Your partner was not just company — they were part of your regulation

To make it clear why the body’s response is so fierce, the two of us first need to understand what a long-term partner actually is for the nervous system. Science’s answer: not an external add-on to your equilibrium — a constituent part of it.

Attachment theory, extended to adult relationships by modern psychology, says that a partner in a long relationship becomes a primary attachment figure: the person your system settles beside, the one your sense of safety is calibrated to. The researchers David Sbarra and Cindy Hazan (2008) described this mechanism with the concept of co-regulation: over the years, partners quietly regulate one another’s physiology — heart rhythm, stress hormones, sleep, even body temperature. You fall asleep more easily together; stress drains away faster in the presence of a familiar voice; a hug after a hard day is not symbolism but a physiological intervention.

This is not a poetic metaphor. In a well-known study by James Coan and colleagues (2006), married women lay in an MRI scanner awaiting mild electric shocks: when they held their husband’s hand during the wait, their brain’s threat centres activated significantly less — more so than when holding a stranger’s hand, and far more than with no support at all. The closeness of a trusted person literally lowers the load the nervous system has to handle on its own.

Now subtract that person. A system that has worked as a pair for years must regulate everything alone overnight — and that is precisely the feeling of being “out of tune”: you are not. You have lost the outer half of your thermostat. Protest and despair: separation distress follows a script

The response to losing an attachment figure is not chaotic — it has an old, well-described course. John Bowlby, the father of attachment theory, observed it in small children separated from their parents, and the neuroscientist Jaak Panksepp showed that mammals have a separate, ancient brain system for separation distress that overlaps with the system for physical pain.

The first phase is protest: the system raises the alarm and drives towards reunion. Hence the restlessness, the insomnia, the constant checking of the phone, the impulse to write, to call, to drive past. This is not a lack of pride — it is the activation of one of the oldest programmes we possess, the same one that makes a lost child cry out for its mother. The second phase is despair: when reunion doesn’t come, the system switches to low energy — numbness, listlessness, withdrawal. This too is a programme, not a weakness of character.

Here the two of us stay with the body: what matters is that you recognise protest and despair as phases that do run their course — not as proof that you cannot live without this person.

A practical conclusion for the protest phase follows from this: the impulse of “just one more message” is not the voice of truth about the relationship but the voice of an alarm looking for quick relief. Every contact eases the system for a moment — and prolongs the protest, because reunion becomes possible again. If at all possible, give yourself a period of no contact after a separation, or limit it to essential logistics; you are not taking away the system’s hope, you are giving it the clarity without which it cannot begin to rearrange itself.

Stress, sleep, appetite, immune system: the body in a state of emergency

So what exactly does the body do in the weeks and months after a separation? Some of the most common changes — and their logic:

  • The stress axis works overtime. The system reads the loss as a threat, so there is more cortisol and adrenaline: a pounding heart, tightness in the chest, shallow breathing, tense muscles, waves of heat or cold. The body is on standby as if danger were lurking round the corner — because in a sense, for the body, it is: its safeguard has vanished.
  • Sleep falls apart. You struggle to fall asleep, you wake early, and the bed — shared for years — is suddenly the loudest reminder of the loss. An aroused system keeps watch at night; if you are waking at three, I describe the mechanism in more detail in the piece on waking in the night.
  • Appetite disappears or runs wild. Acute stress suppresses digestion (hence the clenched stomach and the melting kilos), while under prolonged stress cortisol increases the craving for quick, comforting food. Both directions are the same mechanism with two faces.
  • The immune system weakens. Janice Kiecolt-Glaser’s group has shown in a series of studies that marital conflict and divorce measurably alter immune function — separated and divorced participants had poorer immune markers than married ones. The colds, cold sores and inflammations that stick to you in the hardest months are no coincidence.
  • Concentration fails. A system on alarm gives priority to survival, not to meetings. The paragraph you are reading for the third time and the forgotten passwords are an expected side effect — not early dementia.

All of this is an acute response and as a rule it eases as the system learns a new balance. But precisely because it is a genuine physiological burden, it is worth taking seriously — as seriously as you would take recovery after an operation.

A “broken heart” is not only a metaphor

How seriously the body takes loss is shown by an extreme and, fortunately, rare case: cardiology knows takotsubo cardiomyopathy, also called broken heart syndrome — a condition in which severe emotional stress (often the loss of someone close) triggers a sudden, temporary weakening of the heart muscle, with symptoms almost indistinguishable from a heart attack. A large international study (Templin et al., 2015) confirmed that emotional triggers are common in this syndrome and that it mainly affects women after the hormonal changes of midlife.

I am not writing this to frighten you — the likelihood of anything like this happening to you is small, and tightness in the chest after a separation is in the great majority of cases “only” muscular tension and stress physiology. I am writing it because this extreme case makes a point that holds for your more ordinary symptoms too: the link between loss and the body is real, measurable and deserving of serious treatment. (And of course: with serious chest pain, always see a doctor first — not least so that you can be reassured.)

What the system lost — and where it can find new support

To gather everything so far into one picture: with the separation, your nervous system lost a whole range of quiet, everyday sources of regulation. The good news is that for each of them there is a replacement support — none of them a perfect substitute, but together they slowly teach the system balance again:

How you really settle the system: from the bottom up

The key insight: an overwhelmed nervous system cannot be calmed with arguments. “It will be all right” and “rationally, it was the right thing” are truths that a system on alarm does not hear. Calming goes in the opposite direction — from the body upwards:

  • Routine as a prosthetic rhythm. A fixed time for getting up (even after a bad night), regular meals, daylight and daily movement return to the body the predictability that the shared rhythm used to provide. It sounds banal; it is the foundation.
  • Breathing with a longer exhale. A lengthened out-breath activates the parasympathetic system — the body’s brake — directly, via the vagus nerve. A few minutes, several times a day, especially before sleep.
  • The body before words. A warm bath, a brisk walk when a wave of panic hits, the weight of a blanket, a hand on your chest — these are all bodily channels to calm. Why working through the body often reaches what conversation cannot, I have explained in the piece on body-oriented psychotherapy.
  • Co-regulation through people close to you. A system that has lost its co-regulator settles fastest in the company of other safe people — in person, not through a screen. Coffee with a friend is not entertainment; it is a physiological intervention. So don’t withdraw, even when you most want to.
  • Careful with self-soothing that takes more than it gives. Alcohol, night-time scrolling and checking your ex on social media give a minute of relief and an hour of alarm. Every time you avoid them, the system learns something new.

And patience: a system that spent ten years calibrating itself to someone does not recalibrate in a month. Measure progress in weeks, not days — and expect fluctuation. A bad day after a good week is not a return to the beginning; it is learning.

How long will the body stay on alarm?

No one can give you an honest calendar, but some orientation does exist. Research on separations and divorces consistently shows that physiological distress is most intense in the first weeks and months and then gradually subsides in the great majority of people; in their reviews, David Sbarra and colleagues find that most divorced people return to their usual level of functioning and health within one to two years of the separation. A smaller number remain under raised stress for longer — most often those whom the separation catches with little support, with ongoing high conflict, or with a loss that has opened older wounds.

It is also important to know that the nervous system does not recover evenly. The typical pattern is stepped: two good weeks, then a sudden bad day with no obvious cause — often at a trigger you don’t even consciously register. Many people read such a day as “nothing is moving”, when in fact the opposite is true: the bad days are becoming rarer, shallower and shorter, and that is the only measure that counts. I recommend a simple exercise: once a week, write down a score from one to ten for your sleep, appetite and energy. Memory lies when you are in distress — it always claims it has been “equally awful the whole time” — while numbers on paper show a trend you would otherwise miss.

And don’t forget the basic maintenance of the system, the first thing to go in a crisis: movement in daylight, enough protein even when your appetite is absent, care with caffeine after midday and with alcohol in the evening. None of this cures the loss — but all of it determines how much reserve your body has to carry it with.

When to be concerned — and where support is

Most of what is described here settles by itself over the months, with routine and support. But take these signs seriously: if after several months sleep and appetite remain in pieces, if waves of panic don’t subside or are escalating, if the distress spills over into hopelessness or thoughts of not being here, or if you notice that you only function with the help of alcohol or sedatives. Then don’t wait — this is no longer a “normal separation”, this is a system that needs help from outside.

In my practice I offer support through separation for exactly this period: a space where the two of us work not only with the story of the separation but also with the body that carries it — with sleep, with the alarm in the chest, with lost balance. Because a separation is not only something that happened in your life; it is something that happened to your nervous system. And nervous systems heal — slowly, in a safe relationship, in the same way they learnt to be hurt. The body that keeps watch today is the same body that will one evening know how to settle by itself again; until then, you are allowed to help it.
